AI for physiotherapists: bookings, reminders and patients

AI for physiotherapists: sessions at your chosen frequency, rebooking missed appointments and home exercises. What to automate and what to keep human.

AI scheme for physiotherapists: the assistant linked to agenda, sessions, calls and patients' questions of a physiotherapy study

The AI for physiotherapists above all it serves to hold together the treatment cycle: the sessions to be framed with the same frequency and the same therapist, the reminders that avoid the holes between appointments, the exercises that the patient has to do at home between sessions. It is a work of calendar and constancy, which an assistant connected to management software can follow day by day without getting tired.

The rehabilitation program remains yours. The professional profile entrusts to the physiotherapist the elaboration of the rehabilitation programme and the verification of the ‘respondences of the rehabilitation methodology implemented to the functional recovery objectives' (D.M. 741/1994). The assistant remembers, picks up and reports; does not change an exercise and does not evaluate a pain.

In summary

  • In physiotherapy the unit of work is the cycle, not the single session: the assistant book series of appointments with the frequency decided by the physiotherapist.
  • A skipped session is retrieved inside the cycle, if the plan provides for it, instead of finishing at the end of the calendar.
  • Before the evaluation the assistant collects prescription and reports and connects them to the patient's card.
  • The Exercises at Home they remember and register; pain, doubts and changes always pass to the physiotherapist.
  • Since 2022, physical therapists have had a Own Order, and to exercise you need registration in the register: clinical responses come only from those who are enrolled.
  • The telerehabilitation is a healthcare professional's performance; the automatic reminder is secretarial.

A ten-session cycle is not ten appointments

Those who book a physical therapy session rarely book only one. The patient arrives for a cycle, and the value of the cycle depends on the regularity: the same frequency, possibly the same therapist, spaces and equipment available when needed. At the secretariat this results in an interlocking that changes every week, especially with those who work on shifts.

An AI assistant works well on this interlocking if he receives the plan already decided. The typical flow, from the first phone call to the last session:

  1. First assessment. The patient books and loads prescriptions and reports on the channel indicated by the study; the assistant connects them to his card.
  2. Physiotherapist's plan. The therapist decides and records the number of sessions, frequency, any physical therapies and duration of each appointment in the business management software.
  3. Series of appointments. The assistant proposes the entire series with the same therapist, in the times that the patient can keep, and blocks it in the agenda.
  4. Reminders and recoveries. Before each session starts a reminder; if the patient jumps, the assistant proposes a recovery compatible with the frequency of the plan.
  5. Closure of the cycle. The assistant proposes reassessment only if the physiotherapist has indicated it in the plan.

Think of a study with three physical therapists and a shared gym. A knee operated patient has a two-session plan per week and works on shifts that change every Monday. The assistant asks him for the next week's shifts, proposes two hours with his therapist and, if there are none, offers a colleague only if the holder has written that for that plan is admitted. The secretariat intervenes when there is no solution, not for every shift.

In addition to the therapist, the interlocking is about spaces. A gym with a few stations, a device for physical therapies shared between three professionals, a group course with the seats counted: these are constraints that the secretariat knows by heart and that the assistant must find written in the management software, otherwise it proposes schedules that on paper are free and in the studio not. For groups the same rule applies as for individual sessions, plus the waiting list per seat: when someone cancels, the place goes to the first on the list that has the plan compatible with that activity.

Before evaluation: prescription, reports and waiting list

The physiotherapist performs the interventions "independently, or in collaboration with other health figures", and in the same profile works "in reference to the diagnosis and prescriptions of the doctor". For this the first useful thing that an assistant can do is collect the documents before the appointment: resignation letter, reports of instrumental examinations, prescription if there is. They arrive in the patient's card, not in the chat, and the therapist finds them already sorted when he opens the evaluation.

They are data related to health, a particular category for the Article 9 of the GDPR: the assistant only asks for the documents that the physiotherapist has indicated as necessary, not "all you have". The general rules on where the data ends when a provider treats them are in AI and corporate privacy.

The same card serves for the waiting list, which in studies with many post-operative patients weighs as much as the schedule. Here the Italian AI law is clear: in health care the introduction of these systems ‘cannot select and condition access to health benefits according to discriminatory criteria' (law 132/2025, art. 7). Translated for study: Those who pass first establish written criteria by the physiotherapist, such as the date of the surgery or the indication of the surgeon, and the assistant applies them in the order in which the requests arrive. He does not infer them from age, work, or likelihood of cancelling.

The exercises at home: remember them yes, correct them no

In between sessions, the patient does the job, at home, with the program you gave him. It is the point where an automatic assistant is most useful and at the same time most risky. Useful, because it can send the written program or the video recorded by the physiotherapist, remember the exercises in the agreed hours and ask for a simple match: facts, not facts, facts with annoyance. Risky, because the patient answers with questions that require clinical judgment.

Pain is not a question for the assistant If the patient writes that an exercise hurts, swelling has increased or they have lost strength, the assistant does not propose alternatives or provide reassurance. It records the message, puts it at the top of the physiotherapist’s queue and, if there are signs of urgency, advises contacting a doctor or the emergency services. The physiotherapist defines the words that trigger escalation, not the software provider.

An example: a patient in rehabilitation for a shoulder receives the reminder every evening with the video of her exercises. At the end of the week the physiotherapist finds a summary on the card: exercises done in four days a week, an annoyance reported on Wednesday, a question about the resumption of swimming. The question solves her at the next session; the summary only avoided him from reconstructing the week by heart.

Be careful not to confuse this with telerehabilitation. The National Indications Approved in State-Regions Conference on November 18, 2021, they define it as the distance provision of rehabilitative benefits and specify that it is a health activity pertaining to health professionals, with objectives set in the rehabilitation project and a verification of the patient's skills in the use of platforms. The automatic reminder is before and after; the remote session is yours.

The most convenient channel for programs and exercises is often not chat but a patient area: a web app that can be installed on the phone, with the push notifications instead of messages. When it's best to build it and when it's enough ready-made software is discussed in Custom PWA or software ready.

When the patient disappears mid-cycle

In a physiotherapy study single termination is an agenda problem; two cancellations in a row, or reminders that remain unanswered, are often something else: the patient is dropping out of the cycle. An assistant who merely repeats schedules treats the second case as the first, and sends empty messages.

It's better to give him a different rule. After the number of skipped sessions you decide, the assistant stops proposing recoveries and notifies the physiotherapist with a summary: sessions done, sessions skipped, last feedback on the exercises. At that point it is the therapist who calls the patient, because the reason can be clinical (the pain has passed, or has worsened) and it is precisely the verification of the results with respect to the objectives that the professional profile entrusts to him. If the patient writes that he no longer wants to receive messages, the assistant notes it and stops, without insisting.

The same summary is useful at the end of each cycle, even when everything is fine: the physiotherapist comes to the reassessment knowing how many sessions have really been done and how much the patient has followed the program at home.

Register, Order and profile: the stakes of the profession

The professional frame of the physiotherapist has changed in recent years, and those who set up an AI assistant must know it. In order to practice a health profession, ‘in whatever legal form it is carried out,’ registration in the respective register is necessary (D.Lgs.C.P.S. 233/1946, art. 5). With D.M. 183/2022, in force since 15 December 2022, physiotherapists have their own Territorial Orders and a National Federation, separate from those of the other professions of rehabilitation.

Reference What it sets out What it means for the AI assistant
D.M. 741/1994 The physiotherapist processes the rehabilitation program and verifies its results Do not change exercises, frequency or number of sessions
D.Lgs.C.P.S. 233/1946, art. 5 To exercise you need registration in the register Clinical responses come only from registered physiotherapist
D.M. 183/2022 Establishing the Orders of Physiotherapists The reference for register and deontology is the Territorial Order
State-Regions Agreement of 18/11/2021 National indications on telerehabilitation The remote sitting leads the professional
Law 132/2025, art. 7 No discriminatory criteria, right to be informed Waiting lists with written criteria; the assistant presents itself as AI

The second line has a practical consequence. If a patient asks the assistant, "Are you the physical therapist?" or "can I do this exercise even with my knee swollen?", the correct answer is that he is writing to an automatic study system and that the question comes to his therapist. A clinical indication given by a system that is not entered in any register, with the name of the study above, is precisely what the obligation to register in the register serves to avoid.

The last line applies to every conversation: the patient must know that on the other side there is an automatic system. It is also an obligation of the AI Act for those who put into operation an assistant who dialogues with people; the complete picture is in AI Act: The Business Guide.

What to prepare before activating it

An assistant for a physiotherapy study makes as much as the material you give him. Before starting it is best to have it ready:

  • a plan template in the business management software, with the number of sessions, frequency and rules for make-up sessions;
  • approved exercise programs, in text or video, related to individual patients;
  • the list of signals that immediately pass to the physiotherapist, written by him;
  • the criteria of the waiting list, with the date from which they start;
  • the updated information and a welcome message that clearly says that an AI responds.

If the software you use does not handle appointment series and make-up sessions well, the comparison between Custom CRM and ready-made business management software helps to understand if it is better to change it or build around it. End-of-cycle reminders and revaluations follow the same logic as automatic follow-up, with a tone of health study and not commercial campaign; for secretarial, reports and cancellations in an outpatient clinic with multiple specialists see AI for Medical Studies.

The general framework for private health is on the page AI for Private Health. If you want a patient area with cycles, exercises and reminders connected to your management software, Giallo Studio He custom-designed it and tested it with a single therapist before opening it to the whole studio.

FAQ

Can the AI assistant modify an exercise if the patient says it hurts him?

No. The professional profile assigns the physiotherapist responsibility for developing the rehabilitation programme and checking results against recovery objectives. The assistant records the report, places it at the top of the physiotherapist’s queue and, when there are signs of urgency, advises contacting a doctor or the emergency services.

How does the assistant handle a skipped session in the middle of a cycle?

It proposes a replacement session in line with the frequency specified in the plan, for example within the same week, only if the physiotherapist has provided for this. If a replacement is not possible, it alerts the therapist, who decides whether to add a session, move the reassessment, or leave the course unchanged.

Do you need a doctor's prescription to book a first evaluation?

It depends on the case and how the service is provided. The physiotherapist works independently or alongside other healthcare professionals, also taking account of the doctor’s diagnosis and prescriptions: the assistant still asks for available documents and attaches them to the patient record, while the decision about how to proceed remains with the physiotherapist.

Do physiotherapists have their own professional order?

Yes. Ministerial Decree 183/2022 established the Territorial Orders of the health profession of physiotherapist and their National Federation, in force since December 15, 2022. In order to practice a health profession, in any form, it is necessary to enroll in the respective register.

Is an automatic reminder of the exercises telerehabilitation?

No. The national indications approved in the State-Regions Conference define telerehabilitation as a health activity pertaining to health professionals, with objectives and modalities decided in the rehabilitation project. The reminder is a secretarial service; a remote session is conducted by the physiotherapist.

Can the assistant decide who gets priority on the waiting list?

Only by applying criteria written by the physiotherapist or practice owner. Law 132/2025 states that introducing artificial intelligence systems into healthcare must not select or condition access to healthcare services using discriminatory criteria.

Does the patient need to know he's writing to an AI?

Yes. The AI Act calls for people to be informed when interacting with an artificial intelligence system, and Act 132/2025 gives the data subject the right to be informed about the use of these technologies in health care. Just say so in the first message and in the practice’s privacy notice.
